Third Party Consent – Accessing another patients records

We have very strict policies to adhere to on the access of another patients records, regardless if you are a family member.

Access to patient records is not just looking at the records but also inquiring about appointments, medication and anything else to do with the patients medical care.

If you require access to another patients notes they MUST fill in a third party authorisation form and return it to the surgery IN PERSON bringing photographic ID for our staff to verify. Click the below link for a copy of the form

Third Party Consent

If the patient is unable to come to the surgery due to being housebound, the patients carer can attend with photographic ID and bring the form.

If the patient suffers with dementia – we will require proof of Power or Attorney for Health.
