We are happy to welcome new patients to St Paul’s Clinic.
Registering with us is simple. If you need help at any stage, our team will support you.
Who Can Register
You can register with us if you live in our practice area. (Outlined in red below)
If you are not sure, please contact us and we will check this for you.

How to Register
To register, please complete:
- A GMS1 Registration Form
- A New Patient Questionnaire
You can complete the forms online or download them and send them to us.
Send completed forms to:
patient.support.w93039@wales.nhs.uk
We aim to process registrations within 48 hours.
Identification (ID)
If you have ID or proof of address, please include it with your forms.
If you do not have ID, you can still register. We are a “SAFE SURGERY” Safe Surgeries – Doctors of the World
We do not refuse registration if ID is not available.
Need Help Completing the Form?
We understand that forms can be difficult.
If you need help:
- We can support you to complete the form
- We can arrange an interpreter
- We will take the time to make sure you understand
Please contact us and we will help.
Language Support
We support patients who do not speak English as a first language.
- Interpreter services are available
- We have a Language Line video system available in the surgery
- We will take time to make sure you are understood
You can ask for an interpreter when registering or booking.
What Happens Next
Once we receive your forms:
- We will review your registration
- We will contact you within 48 hours
- You may be offered a new patient health check
Once registered, you will be able to access our services.
Important Information
- You do not need proof of address or immigration status to register
- You can register even if you are staying temporarily in the area
- All patients are treated fairly and with respect
